Change in HIPAA Status – download this packet to change the Inclusions or Exclusions section on your original HIPAA Privacy page. This will allow you to control who can be included in your list of people that we may talk with about your health as well as those to whom we are not permitted. NOTE: you cannot exclude your insurance company if you want us to file insurance on your behalf.
Change in Demographics – download this form to change address, phone number, email address, marital status, etc.
Medical Records Request – download this form to request medical records from your other physicians to send to our office.
